Norway’s skiing secrets | Mariella Frostrup [ BeritaTerkini ]

Mariella Frostrup in Norway

After trying her hand at dog sledding, Mariella takes a break. Photograph: Observer

The only sound as I slide along is the whoosh of metal blades on ice and the panting of the huskies. In every direction stretches a dazzling canvas of snow and pine with not a sign of habitation. We are intruders in this winterscape. Johan, the owner of the dog-sledding outfit, is our outrider, leading the way, surefooted as a deer as he careers ahead. Negotiating the bends between tree trunks at breakneck speed without the sleigh toppling brings shrieks of delight from the children. Three hours’ flight away to the south the slopes of the Alps are crawling with humanity, but here in my native Norway we’re far from that madding crowd.

I gave up trying to find our destination, Hemsedal, in the Lonely Planet guide. As far as I was aware it had been one of the country’s most popular downhill ski resorts since the early 60s. Geilo has made a slightly bigger name for itself on the cross-country circuit, but that still didn’t explain why a forensic search yielded nothing. But, though this area is renowned locally as the “Scandinavian Alps”, such reticence to boast about their assets is typical of the Norwegian approach to life.

The pared-back nature of daily life in a country boasting the third-largest sovereign wealth fund is quite a surprise. A predilection for bling in similarly oil-rich nations from Saudi Arabia to Iraq has kept London’s Park Lane car dealers in business through three recessions. Not so the Norwegians, who favour a Volvo from neighbouring Sweden. Instead of squandering their North Sea bonanza they’ve invested it, ensuring future generations enjoy the high standards of free education and healthcare that are the envy of the world. There are few countries where adults still favour “family bunks” and standard bath towels are the size of dishcloths.

Thankfully there’s a newfound thaw in my countrymen’s icy resolve. Where once pine-clad apartments with strip lighting were the order of the day, our accommodation in the hamlet of Totteskogen offered luxury. Five minutes from the children’s slopes and set among a cluster of cabins, our home for the week, called Dinabu, was a medley of stone and wood, with floor-to-ceiling glass framing the mountain view, underfloor heating and not one but two log fires along with a small sauna and a huge boot room to put wet ski gear in to dry. Privately owned mountain retreats like ours are let by their owners only for a proportion of the year, so personal touches are everywhere.

Hemsedal

Among downhill aficionados the word “flat” often finds its way into descriptions of Norway’s skiing potential, yet as I sat on our sunny terrace surrounded by precipitous pinnacles I put such comments down to malicious propaganda. Hemsedal doesn’t boast the challenges of the Alps – of the 45 runs the majority are green and blue, but there are enough reds and blacks to keep my fanatical husband and children happy.

My favourite was a long, leisurely blue, winding its way from the top of the mountain, that took a good 30 minutes to ski and was virtually empty until the lower slopes. The only company on the descent was the whistle of the wind, the swish of my skis and the endless anecdotes of my seven-year-old son, who has an unparalleled ability to gabble and ski simultaneously.

Despite my Nordic genes I’ll never be a champion skier. I’m brimful of fear, with not a gung-ho vein in my body, but my family are a different matter. I realised a couple of years ago during our debut ski trip that I was set to be a ski widow. Molly and Dan, five and six at the time, couldn’t wait to hurtle down the slopes at terrifying speed. Such was their aptitude that my husband took them down their first black run on that trip. Luckily I only found out about it afterwards. My own earliest ski memory dates back to my kindergarten sports day just outside Oslo. I was five years old and lost courage halfway down the snow steps carved into the slope for our “fun run”. I sat down and wept, and the snickering of my classmates still echoes down the decades.

On a trip to the town’s supermarket, we stocked up on local cheeses, fresh eggs, pickled herrings, rye bread and other northern culinary staples for breakfast and lunch. Thanks to our gifted chef Karl, who turned up each evening to whip up local delicacies, from halibut with roast pepper and crème fraîche coulis to spring chicken with a purée of root vegetables, we had little compulsion to set foot outside after dark. Instead we curled up by the fire playing a game of gin rummy.

The focus on daytime family fun, from dog sledding to heart-stopping snowmobile racing on a circuit high up on a mountain plateau, along with a host of English-speaking instructors to teach the kids at ski school, proved more than enough to keep us occupied.

Until this year relative inaccessibility (a four-hour drive or ski-bus transfer from Oslo airport) diminished Hemsedal’s appeal. A new flight from Gatwick to Fagernes airport, Leirin – with a 75-minute transfer – changes that, but hopefully won’t spell the end of the empty slopes we enjoyed.

After a week in the mountains we stopped off in Oslo for a night and discovered a lurch toward sophistication and development that is transforming the provincial city of my childhood into a hub of art, design and culinary experimentation. A dockland development, Tjuvholmen (Thieves Island, because it used to be populated by, you guessed it, thieves) faces out to the archipelago, with a boutique hotel, the Thief, along with restaurants, coffee bars, ice-cream stalls and shops, culminating in a surreal manmade sandy beach littered with contemporary sculptures. The latest addition to this sprawl of low-rise modernity is the Astrup Fearnley Museum of Modern Art, a symphony of glass and wood designed by Renzo Piano, where Damien Hirst’s butterflies and Tracey Emin’s tapestries hang among work by YSAs (Young Scandinavian Artists).

In Britain, we list perpetually toward the equator, convinced that we have more in common with our happy-go-lucky, laconic and increasingly bankrupt neighbours than our less-effervescent cousins towards the Arctic Circle. Yet as Scandinavia slowly conquers the globe with crime novels and TV series, design stores and fashion chains, you can’t help wondering what other temptations they’ve been hiding from us.

Mariella’s holiday was provided by visitnorway.co.uk. For more information on Hemsedal, go to hemsedal.com. For details of holidays, go to ski-norway.co.uk. Norwegian Air (norwegian.com) flies directly from various UK destinations to Oslo, with flights from £39 one way. SAS (flysas.co.uk) also flies directly from various UK destinations to Oslo. A new charter will run every Sunday from 20 December to 20 April. Book with Crystal Ski (crystalski.co.uk)

Key U.S. official faces grilling over how Obamacare website became fiasco [ BeritaTerkini ]

By David Morgan and Susan Cornwell

WASHINGTON (Reuters) – Two months before the troubled October 1 launch of Obamacare exchanges, a key administration official overseeing the program assured a congressional oversight panel that work was on track to roll out a tested website that would make it easy for Americans to enroll in affordable health insurance coverage.

“This is a large and complicated endeavor that I am proud to lead, and every decision is being made by my prior work experience,” Marilyn Tavenner testified on August 1 before the House of Representatives Energy and Commerce Committee, describing the launch of the Healthcare.gov website.

Come Tuesday, the former nurse who heads the U.S. Centers for Medicare and Medicaid Services (CMS) will again find herself before a House committee – this time, to explain how Healthcare.gov failed when the administration flipped the on switch. She will face Republicans eager to prove, thus far unsuccessfully, that the White House orchestrated decisions that may have stalled the system.

Lawmakers on both sides of the partisan aisle are growing increasingly impatient with website snafus that they say are frustrating the public and adding to taxpayer costs. The White House has scrambled to fix technical issues and disputes Republican allegations that political motives were behind changes in the website’s function.

Tavenner’s scheduled testimony before the House Ways and Means Committee is expected to offer insight into the decision-making. A key player, she was cleared to visit the White House 425 times between December 2009 and June 2013, including for several meetings with Obama himself, visitor logs show.

One Oval Office meeting with Obama in March would have occurred as some technology officials in her agency publicly fretted about the possibility that the complicated website would malfunction, telling an insurance forum they were working to avert problems.

Tavenner, 62, who was confirmed for her job by the Senate in May, was optimistic about the rollout when questioned by skeptical Republican senators at an April hearing.

Tavenner is expected to be a critical witness this week because “she’s more responsible for decisions made at CMS that probably led to this disaster,” said Joe Antos, a healthcare analyst with the conservative American Enterprise Institute think tank.

A committee aide, who spoke on condition of anonymity, said: “We expect her to be forthcoming. We think she’ll be a very serious witness, and she’s certainly integral.”

Tavenner appears one day before her boss, U.S. Health and Human Services Secretary Kathleen Sebelius, is due to testify before another panel in the Republican-controlled chamber.

Committee aides hope that Tavenner can describe system problems at the more complicated back end of the federal marketplace, where consumers determine their eligibility for premium subsidies and enroll in coverage. Aides and experts fear new crippling problems could emerge as enrollment picks up in November and early December.

LAST-MINUTE DECISION

There is also intense interest in Washington in learning who decided at the last minute to deny visitors to Healthcare.gov the ability to browse insurance plans without first creating a website account. That decision is widely blamed for the bottlenecks that helped paralyze the system as millions of visitors flooded the marketplace in the first days of enrollment and during the ensuing weeks.

“That (decision) had to be made at the highest possible levels, meaning in my view the White House. That’s a strategic call about selling the reform,” Antos said.

White House visitor logs, which provide a public record of who visits with administration officials, have not yet been released for the August period when potential problems with the website launch may have been discussed.

Republicans also want to know who in the administration decided to make Tavenner’s agency the “quarterback” or system integrator for the huge information technology system behind Healthcare.gov. Analysts say that decision – rather than giving the job to the private sector – also may have created problems.

Last week, the administration announced that it was handing the job over to a private contractor as part of the effort to fix the online enrollment system.

CMS, the agency that oversees the massive federal Medicare and Medicaid programs, already had plenty to do before it took charge of implementing Obamacare, the Senate’s leading Republican Mitch McConnell said in May, after voting against Tavenner’s confirmation.

Tavenner, who had served as acting administrator for more than a year, was nonetheless easily confirmed by the Senate on a 91-7 vote. Promising to run the agency like a business, she won accolades from leading Republicans who looked favorably on her career as a nurse and later as an executive for Hospital Corporation of America. She left HCA after 25 years to become Virginia’s health and human resources secretary.

Republican House Majority Leader Eric Cantor, a fellow Virginian, introduced Tavenner at her Senate hearing. He said he differed with Obama’s healthcare policy, “but if there is anyone that I trust to try and navigate the challenges, it is Marilyn Tavenner.”

House Republican lawmakers at Tuesday’s hearing are expected to focus not just on the healthcare website, but on the Affordable Care Act and its impact, aides said.

“The website is terrible … but the real problem is the law, which is causing people to lose coverage that they already have,” one Republican aide said.

Democrats will ask Tavenner what steps the administration will take to fix the reported website problems, one House Democratic aide said.

The Democrats may focus on positive experiences of some of the 700,000 people who have filled out applications as a first step toward enrollment, including some who have been denied insurance previously because of pre-existing conditions, the Democratic aide said.

Nonetheless, Democrats view the hearing as a largely political event staged by Republicans as part of their continued criticism of Obamacare, he said.

On Friday, aides to committee Republicans were reviewing what Tavenner said on the record to Congress about the healthcare website before it went live, and comparing that with the actual rollout.

Turn to herbal relief to help ease chronic nerve pain – Tulsa World [ BeritaTerkini ]

Dear Pharmacist, I have nerve pain that is chronic and related to my diabetes. I take medication for that, and want to add some supplements that might help. I promise to ask my doctor if they’re alright for me. — P.T., Tulsa

Nerve pain is termed “neuropathy,” and sometimes you see it as “peripheral neuropathy.” It can be described as tingling, burning, radiating and sharp. Everyone’s experience is different.

Diabetes medications can sometimes exacerbate neuropathy by causing a drug nutrient depletion. Some of the most popular medications prescribed (e.g. metformin, glipizide) are “drug muggers” of vitamin B12. You need B12 to produce myelin, a protective fatty coating around your nerve fibers. Your nerves get touchy and neuropathy can begin if you run out of myelin. Supplementing with methylcobalamin might help, but do test to see if you are low in that. It’s a blood test. There is more about nerve-soothing remedies in my “Diabetes Without Drugs” book.

Herbs that are in the “nervine” category can be very nourishing and soothing to the nerve tissue. Among the best are Chinese skullcap, lemon balm, wood betony, St. John’s wort, chamomile, prickly ash and milky oats. These are found in a variety of ways including commercial tea, dried herb so you can make your own tea or compress, tinctures, capsules and so forth. They each have a book full of side effects and precautions. Do not take it upon yourself to just self-treat without seeing a knowledgable practitioner who studies and prescribes herbs for a living.

For milder effects you could always take a bath in herbs; mix together the following to make two cupfuls: oatstraw, skullcap, wood betony and St. John’s wort. Put it in a clean sock and drop into your bath. You can also add five to 10 drops of lavender essential oil. Soak for at least 20 minutes, keeping the water lukewarm. This must be discussed with your practitioner as there is transdermal absorption of these herbs. If you have a local (small) area, you can also try a commercial product called Neuragen sold at pharmacies nationwide.

The Disney Resort Shanghai is scheduled to open at the end of 2015. The Disney Store concept debuted in 1987 and now features 340 locations around the world. The store in Shanghai will represent the first store on the Chinese mainland.

Big haul of tainted-herbal – Jakarta Post [ BeritaTerkini ]

Dodgy medicine: Semarang Food and Drug Monitoring Agency (BBPOM) chief Zulaimah displays on Wednesday tainted traditional herbal medicine and illegal drugs, including sex enhancers, confiscated from two distributors in Banyumas and Sukoharjo, Central Java. (JP/Suherdjoko)

The Semarang Food and Drugs Office (BBPOM) confiscated jamu (traditional Indonesian herbal medicine) worth Rp 700 million (US$ 62,300) found to contain chemical substances, in two busts in Banyumas and Sukoharjo, Central Java, from Oct 22 to Oct 23.

BBPOM Semarang chief Zulaimah said herbal medicine, worth Rp 600 million, was confiscated from one distributor — identified by the initials UD — in Banyumas and the rest from EW, a distributor in Sukoharjo.

“We found 165 jamu items that contained chemical substances and drugs without permits,” said Zulaimah in a press conference in Semarang, on Wednesday.

In total, 223,453 items — comprising 143 different types of herbal medicines without distribution permits, 10 types of products with high dose drugs and 12 types of illicit drugs — were seized.

The items confiscated were supposedly used to treat uric acid, rheumatic pain, headache and erectile dysfunction, as well as to increase female libido. They included various brands such as Pollsex, Maxjozz, Macho X, Empot-empotan Plus and Empot Madura Plus.

High dose drugs being used in the herbal products include Ponstan, Supertetra and Neuralgin RX.

The agency’s food and pharmaceutical observation chief, Matheus Kristianto, said the erectile medicines tested positive for tadalafil and sildenafil, which were normally used to treat erectile dysfunction.

“Some producers bought Viagra pills and grinded them into powder for mixture materials in the herbal medicines,” he said.

Last month, BBPOM Semarang destroyed illegal herbal medicines and drugs taken from markets that were worth Rp 1.4 billion. (ebf)

Danish National Maritime Museum – review [ BeritaTerkini ]

What do you do if you want to design an iconic building – a concept which for at least a decade has been declared terminally ill – that is not an icon? What if you are Bjarke Ingels, a walking whoosh of brilliance, ambition and salesmanship dulled only very slightly by near-permanent jet lag, who is currently sweeping the world of architecture in a way few 39-year-olds have managed before? Who, with this project, is returning to his (careful, thoughtful) homeland of Denmark on a site patrolled – as if by cultural drones – not only by history, but also by the greatest work of literature of all time. The answer is a building whose highest point, its glass balustrade, is about a metre off the ground, but which offers underneath a work of infinite jest and most excellent fancy.

The project is the Danish National Maritime Museum, and although it is on a spot surveyed by old fortifications, what makes it keep its head down is not artillery fire, but the fact that this is a Unesco world heritage site. It’s in the town of Helsingør, known to literary Anglophones as Elsinore, and presiding over the scene are the walls and turrets of Kronborg, otherwise known as Hamlet’s castle.

Shakespeare’s use of foreign locations (Illyria, the coast of Bohemia, Bermuda) could be notional, but this wild and whirling promontory plays its part in Hamlet well. It’s easy to think that ghosts and madness lurk in its nipping and eager air, notwithstanding the lugubrious ply of frequent ferries to nearby Sweden. It’s a place of both real potency and ersatz significance – as in the end the connection to the Bard is somewhat arbitrary.

danish national maritime night
‘Ramped, zigzagging bridges encourage visitors to descend in moderate haste.’

It’s an interesting site for an architect who has chosen to call his practice BIG (as in Bjarke Ingels Group), which gives its Danish website the name www.big.dk. If a £34m cultural landmark would be an all-consuming, career-defining, once-in-a-lifetime opportunity for most architects of Ingels’s age, this is almost toy-like compared with the mountain-like blocks of flats he has already built on the edge of Copenhagen, and the 450ft pyramid he has under way on West 57th St, Manhattan… or the 500-metre-plus tower he has designed in Tianjin, China. With this museum he is getting himself to a nunnery.

The original intention for the museum was to fill a 1950s concrete dock, drained of water, but BIG pointed out that it would be more interesting to leave the dock open as far as possible, with the new galleries wrapped around it in subterranean chambers. Interesting, if complicated, as the structure is not only underground but also, being next to the sea, in effect underwater. A triple layer of concrete and steel has to deal with this perilous circumstance, a job that the old dock wall might otherwise have done until doomsday. Also, because the air-filled void would now be buoyant, steel anchor rods had to be screwed into the ground to prevent a strange eruption of the whole building.

As a result, the dock now works well as a hole to keep the wind away. Bounded in this concrete nutshell, what’s more, you find the necessary architectural dazzle. Ramped, zigzagging bridges encourage visitors to descend in moderate haste, first from ground level to the entrance, then through light-filled short cuts between the black-box galleries that ring the outer perimeter. One contains the banked seats of an auditorium, with a stage for lecturers sloping the other way – which, unusually, is given as much floor area as the audience have. It promises to make talks on maritime history into a kind of performance art, albeit one viewed from some odd angles in certain seats – north-west-by-west, upskirt, neck-cricking.

danish maritime museum interior
An auditorium ‘promises to make talks on maritime history into a kind of performance art’.

As you look along the dock from one glass-and-aluminium bridge to another, their reflections and transparencies turn them into objects of infinite space, their layering of multiple uses – circulation, exhibition, auditorium – a combination and a form indebted to Ingels’s former employer, Rem Koolhaas. The contrast of the shiny new stuff with the harsh world of the existing structure is made as strong as possible, and where repairs to the old concrete were required the contractors were instructed to rough-hew them how they wanted. The bottom of the dock, a hoped-for public space of unusual properties, is rough and brusque: steel cylinders, the termini of the 40-metre bodkins going deep into the ground, prod upwards unceremoniously.

This is partly a consequence of money running out, but also part of the Ingels aesthetic. The soul of BIG’s wit is brevity, and you could explain their design concepts between two floors on an elevator ride – it’s a mountain of flats; it’s a ramp for bicycles rising 10 storeys in the air (which is what their “8 House” apartment block in Copenhagan is); it’s a zigzag in a hole. BIG is less concerned with the endless iteration of designing details or making the project thick with thought. They don’t aim to control every surface. It’s not that they can’t be bothered (I spent a long time with the project architect, Jeppe Ecklon, who has spent years of grunt and sweat on getting the museum more or less as BIG wanted), but they are not greatly interested in perfection.

danish museum 3
Photograph: BIG

The result is not resonant. Elsewhere in Elsinore time has given smoothness to unalike substances, such as stone, wood, iron and turf, next to which BIG’s brittle surfaces are jangled, out of tune and harsh. I would prefer a metal more attractive than aluminium, or possibly not metal at all. However, the big move – the diagonals taking you down into the undiscovered country of the old dock – is as apposite as it is bold. It is supported by other moments of invention, such as apparently simple staircases that turn out to descend at different pitches, depending on which side of their broad width you are using. It is not supported, on the other hand, by the exhibition design, by the Dutch company Kossmann.dejong, which feels the need to offer sounds and film of the sea, in case you had forgotten what it is, and which undermines the purposeful perfection of maritime equipment with mannered, angular cases. A wise man has called such desperate appliances, which pre-empt the work of the imagination, “pre-chewed food”.

As Ecklon says, BIG can be rude because so much Danish culture is gentle and civil. If all Danes were like these architects, the country would be unbearable. But there is method in BIG’s badness, as – no matter how wayward their projects might look – there is an intelligence behind their moves. A work like the 8 House, a vast ziggurat-hive, looks horrifying at a distance, but closer-to it has surprising intimacy and richness. And the maritime museum, if I might quote Shakespeare for a moment, is a goodly frame.

‘Fallon’ Wraps Pearl Jam Week With ‘Lightning Bolt’ [ BeritaTerkini ]

‘Fallon’ Wraps Pearl Jam Week With ‘Lightning Bolt’

Eddie Vedder reminisces about ‘Balls In Your Mouth,’ chats with Javier Bardem

Late Night With Jimmy Fallon wrapped up its weeklong Pearl Jam celebration last night with a second and final visit from the band. After performing their new single “Sirens” the night before, Pearl Jam launched into “Lightning Bolt,” the title track off their new album.

Eddie Vedder sat down with Jimmy Fallon and fondly reminisced about the pair’s 2011 duet, “Balls In Your Mouth.” “It’s really an environmental anthem,” Vedder mused. He also chatted with fellow-guest and Pearl Jam fan Javier Bardem, telling a story about Bardem introducing the band at a concert in Spain that left the audience in tears. “I was drunk,” Bardem explained. “I didn’t know what I said, but apparently people didn’t like it. They were crying for the wrong reasons.”

Analysis of Herbal Products Shows Contamination Is Common – Science Daily (press release) [ BeritaTerkini ]

Oct. 10, 2013 — Most herbal products, available to buy as alternative medicines, may be contaminated. Reporting in BioMed Central’s open access journal BMC Medicine researchers demonstrate the presence of contamination and substitution of plant species in a selection of herbal products using DNA barcoding.

There is currently no best practice for identifying plant species in herbal products. Traditionally plants are identified through the appearance of the whole plant. This method is not useful though when analyzing processed plant material. DNA barcoding analyses a short genetic sequence from the plant’s genome and identifies small differences that allows species identification. In this new study the researchers used barcoding to examine the plant species found in a sample of herbal plant products.

The results showed that 59% of the products contained plant species not listed on the labels. Over two thirds of the products tested had plant species present which were a substitution for the plants listed on the label and a third of products also contained other species that may be a filler or contamination.

According to the World Health Organization, the adulteration of herbal products is a threat to consumer safety. In this current analysis the researchers detected plant species that could pose serious health risks when consumed. The results revealed plant species with known toxicity, side effects and/or negatively interact with other herbs, supplements, or medications were present in some products.

The authors concluded that the contamination and substitution dilute the effectiveness of otherwise useful remedies, lowering the perceived value of all related products because of a lack of consumer confidence in them. ‘We suggest that the herbal industry should embrace molecular diagnostic tools such as DNA barcoding for authenticating herbal products through testing of raw materials used in manufacturing products. This would be a minor cost to industry with a limited amount of bulk product testing, which would certify a high quality, authentic product’, said Dr. Steven Newmaster of the University of Guelph and lead author of the paper.

Celgene arthritis drug appears effective, safe after one year: study [ BeritaTerkini ]

By Bill Berkrot

(Reuters) – Previously untreated patients experienced significant and lasting reductions in signs and symptoms of psoriatic arthritis when given Celgene Corp’s experimental oral drug, apremilast, according to data from a late stage clinical trial.

Patients taking either the 20 milligrams or 30 mg dose of apremilast twice a day had a statistically significant improvement in symptoms, such as painful, swollen joints, after 16 weeks of treatment compared with those who received a placebo, researchers said.

Signs and symptoms of the painful disease continued to improve on apremilast through a year of treatment, according to data to be presented next week at the American College of Rheumatology meeting in San Diego.

If approved, apremilast will compete with injected biotech medicines that have proved to be highly effective, but have more serious potential side effects, such as opportunistic infections and tuberculosis because the biologics suppress the immune system.

“Psoriatic arthritis can be one of the most crippling types of arthritis that we see,” said Dr. Alvin Wells, the lead investigator of the 527-patient study who will present the data at the ACR meeting on Tuesday.

“To be able to have a patient that can take a pill twice a day, I think this is going to be a game changer, not only for me as a physician, but for my patients. They’re going to have a new treatment option,” added Wells, director of the Rheumatology and Immunotherapy Center in Franklin, Wisconsin.

The U.S. Food and Drug Administration is expected to make an decision on the drug in March.

Celgene has said sales of apremilast could reach $ 1.5 billion by 2017. Wall Street analysts’ projections have been more modest, with RBC Capital Markets forecasting sales of $ 1 billion in 2017, while Cowen and Co sees annual sales reaching $ 475 million in 2018.

The primary goal of the study dubbed Palace 4 was a 20 percent reduction in signs and symptoms of the disease, known as ACR20, after 16 weeks versus placebo.

At that point, 29.2 percent of those who got the 20 mg dose and 32.3 percent to took 30 mg achieved ACR 20, compared with 16.9 percent on placebo.

After 52 weeks on the drug, 53.4 percent on the lower dose and 58.7 percent on the higher dose achieved an ACR20 response.

“These patients are on drug a long time so I want to see that it’s going to last,” said Wells, calling the results “a pretty dramatic response.”

Placebo patients who did not respond early in the trial were switched to apremilast. By week 24 there were no longer any patients receiving placebo, Wells explained.

Researchers also looked at how many patients experienced improvements of 50 percent and 70 percent.

At 52 weeks, for those on the higher dose of apremilast, 31.9 percent had achieved ACR50 and 18.1 percent ACR70, researchers said.

About 125 million people worldwide suffer from the scaly skin condition psoriasis and about 30 percent of them can develop psoriatic arthritis, a chronic, progressive disease in which joints become swollen and inflamed.

Apremilast is the first drug in a new class of medicine called PDE4 inhibitors.

“It inhibits an enzyme involved in making all these angry proteins that get into the skin, causing psoriasis, and they also get into the joints, causing arthritis,” Wells said.

The most common side effects with apremilast were diarrhea, nausea and headache. No more serious adverse side effects were reported, and fewer than 2 percent of patients in the study discontinued treatment due to side effects.

Quincy Jones Sues Michael Jackson Estate [ BeritaTerkini ]

Producer claims breach of contract, seeks millions in damages and unpaid royalties

Quincy Jones filed a lawsuit against Michael Jackson’s estate on Friday, seeking millions of dollars in royalties over projects that were released after the singer’s death. The legendary producer is claiming that the way Jackson’s work was used in the This Is It film and soundtrack album, as well as the Michael Jackson Cirque du Soleil productions and the 25th anniversary edition of Bad, involve a breach of contract. Jones also included Sony Music Entertainment, the parent company of Jackson’s label, Epic Records, in the suit.

According to The Hollywood Reporter, the producer claims that master recordings of songs he worked on were remixed and edited in order to deprive him of profits that he would have been entitled to under agreements he signed with Jackson in the 1970s and 1980s. He says that under the contracts, he should have been given the first opportunity to re-edit or re-mix any of the master recordings and that he was entitled to producer credit for the master recordings, as well as additional compensation if the masters were remixed.

See Where Michael Jackson Ranks on Our List of the 100 Greatest Artists

Jones produced three huge solo albums for Jackson: 1979′s Off the Wall, 1982′s Thriller and 1987′s Bad. Following the singer’s 2009 death from an overdose of the anesthetic propofol, Columbia Pictures released the film This Is It, which documented Jackson’s preparations for his concert tour. The film and soundtrack featured songs including “Don’t Stop ‘Til You Get Enough,” “Thriller,” “Beat It” and “Billy Jean.” Cirque du Soleil then used a similar array of classic Jackson hits in its 2011 production Michael Jackson: The Immortal Tour and this year’s Michael Jackson: One.

Jones is seeking at least $ 10 million in damages for breach of contract, as well as unpaid royalties, remixing fees and compensation for the loss of credit. “Quincy has been frustrated with these matters for a number of years, felt he was not making any progress and needed to take more formal action,” Henry Gradstein, Jones’ attorney, told THR.

Howard Weitzman, a lawyer for the Jackson estate, issued a statement in response. “The estate of Michael Jackson was saddened to learn that Quincy Jones has filed a lawsuit seeking money from Michael’s estate,” the statement said. “To the best of our knowledge, Mr. Jones has been appropriately compensated over approximately 35 years for his work with Michael.”

Elvis Costello Named Honorary Doctor of Music [ BeritaTerkini ]

Singer tells New England Conservatory audience that he wrote new songs with Bacharach, may release McCartney demos

As if he hadn’t already proved it countless times with his surgically precise songwriting and the healing power of his gift for melody, Elvis Costello can now call himself a doctor of music. On Friday, the ever-restless musician was on hand in Boston to receive an honorary degree from the New England Conservatory, joining a small, illustrious group – Miles Davis, Aretha Franklin and Quincy Jones – of Rock and Roll Hall of Famers who have been honored by the country’s oldest independent school of music.

15 Rock & Roll Rebels

After accepting his diploma, Costello joked that it was appropriate coming from the New England Conservatory: the old England has tried to kick him out from time to time.

Relaxed and witty as ever, the singer sat onstage for a 90-minute retrospective of his career at the college’s historic, acoustically pristine Jordan Hall, backdropped by the venue’s huge pipe organ. Listening to a snippet of “God Give Me Strength,” his elegant 1996 collaboration with Burt Bacharach, he told the audience that he recently wrote 12 new songs “on a little upright piano” with the 85-year-old pop composer.

He also told his interviewer, Boston Globe critic Sarah Rodman, that he expects to someday release all the demos from his periodic collaborations with Paul McCartney. Asked if he ever stopped to marvel at the fact that he was writing songs with a Beatle, Costello noted that he joined the Beatles’ fan club when he was 11 and joked, “Yeah – what are you, crazy?”

The discussion ranged from his youthful nagging of mentor Nick Lowe to his work with the Brodsky Quartet (with whom he plans a performance to mark the 25th anniversary of the 1993 album The Juliet Letters) and his latest album, with the Roots, Wise Up Ghost. Reminded that his bittersweet lost-love song “Alison” is sometimes played at weddings, he said, “It does seem like an odd choice, doesn’t it?” (One well-known actress once told him she thought of his deeply psychotic dirge “I Want You” as the most romantic song in the world, he said.)

Acknowledging McCartney’s insatiable curiosity and his own, he said he believes that’s the proper way to approach music: “Have all the experiences, not just one.”

“Not everything you write is supposed to be on American Bandstand,” said the singer, who wore a denim jacket, a vest and his trademark porkpie hat for the occasion. At one point, he held up an iPod and commented on the pros and cons of new technology. You can carry around the complete works of Ralph Vaughan Williams in your pocket, he noted; in the old days, “imagine how big your trousers had to be to do that.”

Costello also spoke at length about being a third-generation musician. His father, Ross MacManus, was a singer for hire who sang the popular songs of the day. By the Sixties, Costello said, he’d gone “a little paisley around the edges.”

What if Costello’s father had been, say, a milkman rather than a musician? Rodman asked. “My mother would have some explaining to do,” he joked in reply. He also revealed how he learned only recently that his mother, Lilian, was involved in “a minor smuggling ring.” With recordings scarce in England after World War II, she arranged for merchant mariners to bring back jazz records from the U.S. for her clients.

After taking a few questions from the audience, Costello headed to the airport to fly to San Francisco, where he will appear with his wife, Diana Krall, this weekend at Neil Young’s annual Bridge School concert. On Monday, Costello will be on The Tonight Show to help celebrate the release of longtime keyboardist Steve Nieve’s new duets album, ToGetHer.

“The longer it goes on,” he said about Nieve, “the more incredible it becomes to me the adventures we’ve had together.”

Bahrain defends use of teargas following criticism [ BeritaTerkini ]

By Mahmoud Habboush

DUBAI (Reuters) – Bahrain has defended its use of teargas after rights groups criticized the Gulf kingdom’s reported plans to buy 1.6 million canisters of the material.

Local and international rights groups have accused the authorities of using teargas excessively and of firing canisters directly at protesters or into cars and houses where they can cause serious injuries.

“Teargas is non-lethal and it is used appropriately by the police, in compliance with the law and in full adherence with the internationally accepted standards contained in the Bahrain police code of conduct,” the government spokesman’s office said in an email to Reuters.

The tiny island state has been in political upheaval since February 2011, when mass protests led by the mostly Shi’ite Muslim opposition were crushed by the Sunni monarchy.

The government statement was made in response to a Reuters query about a leaked document published by Bahrain Watch, an advocacy group, which appeared to be a tender to supply the Interior Ministry with teargas canisters and stun grenades.

The U.S. government has banned the export of teargas to Bahrain.

“Bahrain’s police forces are using less force than is legally permitted,” the statement said.

It did not specifically address the issue of the reported teargas tender, but did say that protesters’ practices of setting fire to car tires and throwing petrol bombs were more dangerous than the authorities’ use of teargas.

It said more than 2,300 police personnel had been injured and nine killed since the protests began 2-1/2 years ago.

An international inquiry in November 2011 found that 35 people had died during Bahrain’s uprising, most of whom were protesters but also including five security personnel. The opposition puts the death toll at more than 80.

The U.S.-based Human Rights First (HRF) advocacy group earlier this month urged Washington to speak out against the planned teargas shipment.

According to Bahrain Watch, the leaked document came in the form of a tender issued by the Interior Ministry’s Purchasing Directorate. It invited arms manufacturers to supply the ministry with “teargas cartridges and sound grenades.”

In March 2012, the UN High Commissioner for Human Rights said the use of teargas had led to a number of deaths of protesters and bystanders in Bahrain.

Physicians for Human Rights (PHR), a U.S.-based group that campaigns for medical staff working in crisis zones, said in August 2012 that the Bahraini government used teargas indiscriminately, causing severe injuries to some civilians.

Bahrain has denied that charge.

Halloween Candy is NO for Canines! [ BeritaTerkini ]

Here's what our sweet pooches should keep their paws away from!

two bulldogs

No matter how cute they are when they beg – NO Candy!

Chocolate: Code red! Dogs and cats should avoid chocolate like the plague! Chocolate contains Theobromine, which can cause heart, nerve and circulatory problems in our furry pals.

Candy Corn: It may be the quintessential Halloween candy, but it can make for a very bumpy – and messy – ride if your pet gets a hold of some. Candy corn will not kill your pup, but all that sugar and corn syrup will cause a great deal of discomfort. Your canine companion will most likely have a very bad belly ache, start vomiting or have diarrhea. Let your pet rest and make sure he or she has fresh water.

Hard candy: Things like lollipops and other hard candies are no-no's for dogs. They can break their teeth, hurt their jaws or choke on those little suckers. As if that were not enough, all the sugar can cause high blood pressure and heart problems in pets. Notice a pattern?

Nuts: Most types of nuts can be toxic for dogs, especially Walnuts and Macadamia nuts. The effects can lead to hyperthermia, paralysis or even death. Mixed with chocolate, nuts can be even worse, so try to resist the Goobers and Almond Joy this Halloween.

Peanut Butter: Peanut butter, believe it or not, is actually fairly safe for your dog. Hooray! Since it technically does not come from a nut (peanuts are legumes), peanut butter can be a little treat that your pet can enjoy without the risks. However, some dogs may be allergic, so consult your vet before feeding your dog any PB. And as always, know your limits. Since it is high in fat, dogs can eat a little bit of peanut butter as long as it is a once in awhile treat.

Keep your pooch away from the ultimate enemy – sugar. If you want to treat man and woman's best friend , pick up some animal-friendly treats at your local pet store!

After tough week, White House buys time for Obamacare website fix [ BeritaTerkini ]

By Steve Holland and Roberta Rampton

WASHINGTON (Reuters) – President Barack Obama promised on Saturday that his troubled healthcare website was just weeks away from a cure as he struggled to convince Americans he is on top of what has become a self-inflicted wound to his signature first-term achievement.

His administration unveiled a plan on Friday to make Obamacare insurance marketplaces on healthcare.gov – a website riddled with error messages, long delays and bugs – work better by the end of November.

It was the end to an embarrassing week where Obama discovered he had overshot on an Oct 1. promise of a website that would make shopping for health insurance as easy as buying “a plane ticket on Kayak or a TV on Amazon.”

“As you may have heard, the site isn’t working the way it’s supposed to yet,” Obama said in his weekly Saturday address – an understatement after days of reports of people being shut out of the system.

“In the coming weeks, we are going to get it working as smoothly as it’s supposed to,” he added.

Obama had stood firm against Republican attempts to defund or delay the healthcare law, known popularly as Obamacare – efforts that led to a 16-day government shutdown this month.

He and his top officials had warned publicly before October 1 that there could be “glitches,” but the White House has been scrambling to control the damage from a rollout that was far worse than expected.

The depth of the design flaws has raised questions about why the Obama administration was so insistent on starting the enrollments on October 1 when the system was clearly not ready – and laid bare the president’s mistake in raising expectations about how good the website was going to be.

“Either they made assumptions that were too optimistic and were caught off guard, or they knew that the difficulties would be greater than the public understood, but chose not to say so,” said Bill Galston, a Brookings Institution expert who was a domestic policy adviser to Democratic President Bill Clinton.

“It may be some of both.”

CRISIS MANAGEMENT 101

Obama adviser Jeffrey Zients, appointed on Tuesday to figure out how to manage the complicated fixes for the website, was an unannounced participant on a conference call with health reporters on Friday afternoon.

Zients gave a deadline, although he cautioned there was a lot of work to do. “By the end of November, healthcare.gov will work smoothly for the vast majority of users,” he said.

Borrowing from the lexicon of homebuilders, Zients said he had hired a “general contractor” to manage the many contractors on the project, and developed a “punch list” of dozens of problems to address.

The message followed classic corporate crisis management strategy, said Peter LaMotte, a senior vice president at Levick, a firm that devises communications strategies for large corporations and organizations.

“State the facts, be clear, be transparent, and then shut up,” LaMotte said. “That is what we often recommend to our clients.”

That deadline buys the administration and tech experts time to iron out the bugs in the website before millions of Americans give up trying to use it, LaMotte said.

But he added, “People are going to hold you to that date.”

Norm Ornstein, a political analyst at the American Enterprise Institute, said the date was “comforting” because it came from Zients, who is known for being a “straight shooter” with private-sector management expertise.

“He’s not making it up. It does not serve his interests to pick a date without a clue as to whether you can make it,” Ornstein said, noting Zients would become Obama’s top economic adviser at the White House on January 1.

“His credibility is at stake here,” Ornstein said.

POLITICAL BREATHING ROOM

Republicans are using the problems to push for a delay to the requirement that Americans buy insurance by March 31.

“Despite hundreds of millions of taxpayers dollars invested, the website still does not work for most,” Fred Upton, chairman of the House Energy and Commerce Committee, said in the Republican reply to Obama’s Saturday address.

Upton’s panel will grill Health and Human Services Secretary Kathleen Sebelius at a hearing next week.

Zients’ Friday announcement will take some pressure off Sebelius in the hearing, said John Ullyot, who works on crisis communications as a managing director at High Lantern Group.

But Ullyot, a former Senate Republican aide, said the plan would have had more punch had it been presented earlier in the week by the “much louder megaphone” of either Sebelius or Obama.

“They had a full week of drip, drip, drip in the media, and you never want to have that,” he said.

Once the website is fixed, officials will face another communications challenge. They will need to concentrate on luring back to the site those people who gave up trying to access it in the initial phases, said a former Obama administration official, who spoke on condition of anonymity.

“I think that people are going to like the site and sign up for it. The problem is once a user has a bad experience at a website, they’re not usually going to want to go back to it,” the official said.

Don’t be fooled into thinking that big data just relies on te [ BeritaTerkini ]

Grand Bazaar, Istanbul

Even the oldest market in the world uses big data

As big data becomes increasingly popular, it’s occasionally worth taking a step back to think about what companies are looking to achieve as well as the necessary elements. Is investment in new technology really necessary, for example, if there is a cheaper – and ultimately simpler – means of collecting the required information?

Reza Soudagar, writing for multinational software company SAP, eloquently summarised how cost-effective ‘low-tech’ methods can be implemented to collect big data, citing the example of Istanbul’s Grand Bazaar. Housed in Cevahir Bedesten, customers are not asked traditional questions (“Do you need any help?”) but are pressed on more personal matters, such as which country shoppers are from, the duration of their trip and even which hotel (or area) they are staying in.

While one could easily mistake this as an attempt to keep customers interested for longer, a happy side effect, their reasoning is more sophisticated: it’s all about big data.

These questions alone don’t provide enough information to form patterns but staff combine the answers they receive with other traits they can instantly detect – the customer’s age, the kind of watch they are wearing, their smartphone model and even the brand of their clothes re – to match up customers with their likely preferences. And an incredible feature is that this all happens within minutes.

This allows a fairly basic understanding of just how businesses use big data. As Soudagar notes:

[They] gather diverse stores of information to truly understand the context of each customer interaction and then decide on the most appealing merchandise, the best price point or the right marketing offer.

But sometimes, it’s easier to automate – just ask Tesco

It’s also worth noting the experiences of larger stores. When Tesco, Britain’s biggest retailer, originally promised its customers nearly 20 years ago that it would open up a new checkout line if there was more than one person in front of them, the company faced a new challenge.

Their reasoning was simple; shoppers don’t like queues and companies expect that a satisfied customer is likely to lead to return.
In order to complete this new self-imposed challenge, staff working at tills suddenly had one more job; they had to constantly monitor their checkout queue and note how many people were waiting. Meanwhile, those stacking shelves also had to be aware of the length of queues so they could open a new line if necessary.

But although this was cost-effective (in terms of not spending more), one can expect that it led to inefficiencies – staff having to always look over their shoulder or manually input data means they may not have been focussed on their main role.

Eventually, Tesco introduced heat-sensing sensors to monitor the number of people in the store, with the company claiming this led to a 10% increase in pre-tax profits. The technology collected information such as the number of people in each queue, the average wait time and even the average store performance. This allowed Tesco to “predict the checkouts that [needed] to be open in an hour”, according to then CEO Terry Leahy.

By introducing the technology, they managed to automise their big data collection and analyse the trends across all of their stores so they knew how many staff they would need working at any time as well as how many checkout tills to keep open.

Don’t worry if you don’t yet know what to do with all that data you’re collecting – you’re not alone

The world of mobile payments has recently seen a lot of movement. First, Square Inc. made a splash last week by introducing Square Cash, which allows individuals to email money to friends.

This is something that Google introduced to a select set of users earlier this year through it’s Google Wallet programme but the company has yet to roll it out to the masses. The reality? It’s a very smart idea. Having to log on to tedious bank systems to transfer money can be a big hassle and a short email makes sense. All it requires is people to connect their debit cards and send emails to friends, such as the example one below.

Square Cash
Now you can send money to your friends by email. Screenshot: Square.com

Then Dwolla, arguably Square’s biggest rival, announced Dwolla Credit, which means online retailers can receive instant payments from Dwolla users at a fraction of the price they were being charged by credit card companies. All this means that Christopher Mims’s prediction last year, that Square and Dwolla might collaborate, is set to be tested as the final jigsaw piece falls into place.

And within the fast-moving world of mobile payments is Starbucks, who last year hand-picked and invested $ 25 million into Square, with all credit and debit payments being processed by Square across 7,000 stores.

Now thousands of customers are seamlessly paying for their coffee via Square Wallet on their mobile. One less loyalty card to carry around for thousands of Starbucks patrons. But what has the company done with all the data they have collected from their loyalty card scheme? Until last year, very little.

As reported by AdAge, attendees of the Big Data Retail Forum were told by Joe LaCugna, Starbucks’s former director of analytics and business intelligence, that Howard Schultz, the CEO of Starbucks “doesn’t care about data. He has absolutely no head for data.”

But, by March of this year, the company had profiled half of its loyalty card customers, nearly three million people. This allowed Starbucks to better understand its clientele and offer discounts based on specific goals.

Customers who have installed the mobile app and linked it to their loyalty card can be sent offers and rewards if they come in to for a coffee by this weekend, especially if the company worries that they’re unlikely to return soon. And how were regular customers rewarded? By not being offered a discount. After all, LaCugna said, “[why] would we give them a discount?”

And how about Square? Sarah Fair, the company’s CFO told Harvard Business Review that big data was the most exciting trend in the mobile space. “I think we are only scratching the surface of what we can do with data in the right way,” she said. “Big data can be used to create these wonderful experiences that people have opted into, that is incredibly exciting.”

But whether Starbucks is making the most of Square’s potential is yet to be seen and the company is remaining tight-lipped. Upon multiple requests for comment, we were told that no-one at the global coffee company was available to speak to the Guardian.

Do you think small businesses should start introducing technology to collect big data – or should they just follow the example of vendors in the Grand Bazaar? Have your say below or join in the debate on Twitter either with me directly @sirajdatoo or our official account @Guardiandata

Herbal remedy claims require verification – Sarasota Herald-Tribune [ BeritaTerkini ]

DEAR DR. ROACH: I am a woman who just turned 75, and I have a chronic dry cough. I visited my pulmonary doctor, and he diagnosed bronchiectasis. There is no cure, but there are herbal products, such as Creseton. They claim to have a 90 percent cure rate, which is better than no cure at all. Can you give me any suggestions or have you heard about the herbal products? — P.N.

ANSWER: Bronchiectasis is an uncommon lung condition in North America. It is a reaction to previous infection with a scarring process in the small airways. It is similar to chronic obstructive pulmonary disease. It’s more common in women and certain ethnic groups, or in conjunction with conditions like cystic fibrosis or alpha-1 antitrypsin deficiency. There is no way to reverse the scarring in the lungs once it occurs, but exacerbation of the disease can be both treated and sometimes prevented with antibiotics.

I looked up Creseton, and the company reported good results from its own, unpublished study. I can’t recommend the product on the basis of what the maker reported. I would like to believe it, but if something sounds too good to be true, it might be.

DEAR DR. ROACH: I recently read that many doctors and nurses have low-frequency hearing loss, resulting in a falsely elevated blood pressure measurement compared with an automated blood pressure machine. Please comment. — P.I.

ANSWER: Accurately measuring blood pressure is very important, and there are several concerns. For the best accuracy, the blood pressure should be taken seated, with a manual mercury device using a properly sized cuff, three times, and the average recorded (as I learned years ago: thanks, Dr. William Elliott). Only very advanced automated models, costing up to thousands of dollars, can approach the accuracy of a trained clinician.

I couldn’t find the news piece you read, but it makes some sense. Health-care providers aren’t immune to losing hearing as we get older, and hearing loss can lead to inaccuracy in blood pressure measurement. Hearing loss can mean an error of several points.

There is abundant evidence that in the vast majority of cases, home and inexpensive office machines are not as accurate as humans.

DEAR DR. ROACH: There is much controversy circulating about the healthiest choices available today for butter and margarine spreads made with canola oil, coconut oil, flaxseed oil, grapeseed oil and olive oil.

There are so many choices that it is almost mind-boggling to the average consumer. I keep receiving emails from friends and relatives with articles that state margarine was developed to fatten up turkeys, and when that did not fly, it was marketed to people as a healthy substitute for butter. Yet, I read labels on products claiming much lower saturated fat than real butter contains.

Many articles forwarded to me have a product to sell, and so their claims are used push these products, whether valid or not. — M.P.

ANSWER: Today’s margarine is not the margarine of the 1950s. Margarines with plant sterols and stanols (sold as Benecol and Smart Balance) reduce LDL cholesterol, although it’s not clear whether they reduce the risk of heart disease. The main advice I give is to completely avoid trans-saturated fatty acids (on food labels as “trans fat”). Most food companies have received that message, and it’s easy to find high-quality margarine. The vast preponderance of the evidence is that margarines are healthier for you than butter.

That doesn’t mean you can’t have butter occasionally. We all make choices that affect our health. The harm you do from a little butter is small. Too much isn’t harmless, though.
